More Ways to Make Care Accessible
Insurance isn't the only path to receiving care. Lewis Family Wellness Center offers a Sliding Fee Discount Program for clients who are uninsured, underinsured, or experiencing financial hardship.

Sliding Fee Program
Eligible clients may receive free or discounted services based on household income and family size.

Income-Based Discounts
Households at or below 100% of the Federal Poverty Guidelines may qualify for a 100% discount on services. Households above 100% and at or below 200% may qualify for reduced fees.

Simple Qualification
Eligibility is based on income and family size. You do not have to undergo asset testing or apply for Medicaid or health insurance in order to qualify.

Eligible clients may receive free or discounted services based on household income and family size.

Tell Us About Your Household
Complete the application with your household size and annual gross income.

Receive Your Determination
Our team will review your application and provide your Sliding Fee Discount Program determination in writing.

Continue Your Care
Approved eligibility generally remains in effect for 12 months and may be renewed or reconsidered if your financial situation changes.

Lewis Family Wellness Center is committed to serving clients regardless of their ability to pay. Clients who qualify for our Sliding Fee Discount Program may receive reduced-cost or free services, and inability to pay a nominal sliding fee does not automatically prevent a client from receiving services.
